Portugal's SNS (Serviço Nacional de Saúde) provides universal public healthcare funded by taxes. Civil servants (funcionários públicos) may access ADSE — a subsidised supplementary health scheme. Private health insurance supplements the SNS by bypassing waiting lists (which in Portugal can reach 6–18 months for specialists) and providing access to private clinics and hospitals. ADSE members often choose private supplementary insurance for broader coverage.

The 2020 slight dip reflects deferred non-urgent care during COVID-19. Premium growth from 2022 onwards driven by healthcare cost inflation, specialist shortage, rising hospital procedure costs, and growing demand as SNS waiting lists lengthened after the pandemic.

Portugal's healthcare system — SNS, ADSE and private insurance

Portugal has a universal public healthcare system — the SNS (Serviço Nacional de Saúde) — funded by general taxation and available to all residents. SNS covers GP visits (médico de família), hospital care, and emergency services. However, specialist waiting times can reach 6–18 months in many areas, and access to diagnostic imaging and elective procedures is constrained by chronic underfunding and workforce shortages.

ADSE is a special subsidised health benefit scheme for public sector employees (funcionários públicos). It covers a substantial portion of private consultation and procedure costs at ADSE-accredited providers. Civil servants with ADSE often purchase supplementary private health insurance to access providers outside the ADSE network or to cover services with residual co-pays.

Main private health insurers in Portugal (2024)

AdvanceCare (linked to the Luz Saúde / José de Mello hospital group) is a major provider, offering direct access to the largest private hospital chain in Portugal. Other key insurers: Fidelidade Saúde, Médis (Ageas Portugal), Multicare (Fidelidade group), Tranquilidade. Most policies provide access to a network of private clinics (clínicas privadas) and hospital groups such as Lusíadas Saúde, Hospital Particular de Lisboa (HPL), and CUF / José de Mello Saúde.

What private health insurance typically covers in Portugal

Portuguese private health plans range from basic (GP + diagnostics) to comprehensive (full hospitalisation). Standard features of a mid-range plan: outpatient specialist consultations, diagnostic tests (blood work, imaging, ECG), physiotherapy, and emergency care at private facilities. Hospitalisation (internamento) is typically included from “comprehensive” plans upward. Dental (dentária) and ophthalmology (oftalmologia) are usually sold as add-ons or included only in premium plans.

Frequently asked questions

Is this calculator free?
Yes — completely free, no account needed. Nothing you enter is saved.
Can I use private health insurance alongside the SNS?
Yes — private insurance supplements the SNS. You can still use SNS services (GP, emergency, public hospital) at any time. Private insurance gives you the option to bypass long waiting lists and use private facilities where you choose. Many Portuguese residents use SNS for primary care and private insurance for faster specialist access.
What is ADSE and who is eligible?
ADSE (Direção-Geral de Proteção Social aos Trabalhadores em Funções Públicas) is a subsidised health protection scheme for Portuguese public sector workers. It covers a percentage of costs at ADSE-accredited private providers. ADSE is separate from private health insurance — it is a benefit of public sector employment. Civil servants can have both ADSE and private insurance simultaneously.
